Our Policies:
Cancellation Policy
We value your time and do our best to arrange appointments that
work well with your schedule. If you are not able to keep your
appointment, we kindly ask that you provide us with 24 hours
notice. Should you miss three appointments without prior notice
to us, we will not be able to schedule future appointments for
you and appreciate your understanding.
Insurance Payments Summary
We are a fee-for-service practice that accepts all insurance
benefit assignments, but is not a participant in any plan. We
gladly process claims for services rendered as a courtesy to
our patients, however the patient is responsible for his/her
relationship with their insurer. For information on insurance

Financial Arrangements
All financial arrangements are made individually and privately.
In addition to the insurance payment arrangements discussed
above, we accept all major credit cards and also participate
in CareCredit®. Please view our financial policy for more
